Process for factory resetting the Polycom VVX 300 phones in the event that the phones do not already the necessary provisioning information.
Power off the phone.
Power the phone back on.
Wait until you see the cancel soft key appear on the phone screen; press the cancel softkey.
Immediately hold down the 1, 3 and 5 keys at the same time and hold them down for at least 3 seconds while the countdown happens on the next screen after pressing cancel.
The phone will prompt for a password – this is where you will need the MAC address, a.k.a. serial number if the admin password of the phone is unknown.
Key in the MAC address using only lower case letter and numbers – it will reject the passcode is capital letters are used even if what you’ve keyed in is correct.
Remember to use the toggle soft key that changes from numbers to lower case letter to capital letter as you enter the MAC address, a.k.a. serial number.
After keying in the MAC address, a.k.a. serial number, press OK
If you’ve done this correctly, the phone will begin the reset process and reboot
Before the phone completes the reboot, press the cancel soft key again when you see if appear on the screen.
On the next screen, press Setup
The password to access the Setup will be 4-5-6.
In the Setup, navigate to the Provisioning Server by pressing Select.
Select the DHCP Menu by pressing Select again
On Boot Server, Press the Edit soft key
Scroll using either left of right arrows on the direction pad and change the option to Static.
Press the OK softkey when done and then press the Exit softkey to return to the previous screen.
Press Edit again on the Server Type and scroll using either left of right arrows on the direction pad and change the option to HTTP.
Press the OK softkey.
On the Server address, press Edit and key in the following:

(Values such as the colon (:), the forward slash (/), the hyphen (–) and the underscore(_) are hidden underneath the pound (#) key by pressing the key 2 times, 1 time, 10 times and 4 times respectively; periods (.) are hidden underneath the (*) star key with only 1 press of the * key.)

At these following sites, the network switch does not support LLDP so you’ll need to manually configure each phone for VLAN2.

After installing and booting up each phone, follow this sequence to configure:
On Polycom VVX 411 --> Home --> Settings -->Advanced -->243493(this is passcode)à Administration settings --> Network Configuration --> Ethernet Menu --> VLAN Menuà VLAN ID 2 --> Save Config. Reboot the phone.
If you need to confirm the phone IP settings, do following:
Polycom VVX 411 --> Home -->Settings --> Status --> Network--> TCP/IP parameters --> confirm IP (should be higher order 10.2.xx.128 and above).
